Chain of Custody (COC)

Advanced Technology Laboratories issues a unique Chain of Custody form that must accompany samples that are sent to our lab for analysis. For convenience, ATL now has an electronic version in Microsoft Excel available for download. Our clients are able to save themselves lots of time by typing and saving most fields of information, then printing the forms in advance of sample collection. This is especially useful for large projects where paperwork can be extensive. Keep in mind that some fields cannot be edited by computer; items requiring handwritten signatures or for Lab Use Only are locked.
